Bridge of Hope Outpatient Treatment Center is a patient-focused, holistic behavioral health facility located off Sandy Blvd in Portland, Oregon. We serve adults 18 and older who are navigating substance use disorders, co-occurring mental health conditions, or both. Our clinical team is credentialed with over 14 years of hands-on experience in behavioral health, and every care plan is built around the individual, not a one-size-fits-all approach.

What sets us apart is our commitment to wraparound care. That means we are not just treating the addiction or the diagnosis. We offer a full continuum of outpatient care so every client can be matched to the right level of support based on where they are in their recovery. Here is how each level works:

  • High Intensity Outpatient (HIOP / PHP): The highest level of structured outpatient care, involving several hours of clinical programming per day, multiple days per week. Clients return home or to sober housing each evening. This is typically the right starting point for those stepping down from detox or residential care, or for anyone who needs intensive daily structure to maintain early sobriety.
  • Intensive Outpatient (IOP): A step down from HIOP, IOP typically involves 9 to 20 hours of treatment per week. Sessions combine individual therapy, group counseling, and recovery education. It is designed for clients who need meaningful clinical support but are ready to manage more of their day independently.
  • Standard Outpatient (OP): The least intensive level, with fewer scheduled hours per week. OP is best suited for clients who have achieved a solid foundation in recovery and need continued clinical check-ins and support to stay on track over the long term.

Your treatment team will assess which level is right for you at intake and will continue to adjust your care as you progress.

When you first reach out to Bridge of Hope, our clinical team conducts a thorough intake assessment before any placement decision is made. This assessment looks at your full picture, not just your substance use history, but also your mental health needs, living situation, support system, work or school obligations, and any previous treatment experience.

From there, your team recommends the level of care that best matches where you are right now. And importantly, that placement is not permanent. We review your progress on an ongoing basis and will step your care up if you need more support or step it down as you grow stronger in your recovery. Yes. Through our Sober Housing program we operate multiple sober living homes throughout the Portland metro area. These homes are designed to complement your outpatient treatment by providing a stable, substance-free environment where recovery is the shared priority. Available housing options include:

  • Dedicated men’s sober living houses
  • A women’s sober living house
  • An LGBTQ+ affirming sober living house

Clients enrolled in our outpatient programs are welcome to reside in one of our houses. Having stable, sober housing during treatment significantly improves outcomes. It removes the stress of navigating an unsafe or unsupportive home environment while you are doing the hard work of recovery.

Yes, and this is central to how we practice. Addiction and mental health conditions frequently occur together, and treating one without addressing the other significantly reduces the chances of lasting recovery. Our mental health services include treatment for depression, anxiety, PTSD, ADHD, bipolar disorder, OCD, and insomnia. We also offer psychiatric medication management to support clients whose conditions benefit from a medication component alongside therapy.

Every treatment plan at Bridge of Hope is integrated, meaning your mental health care and your addiction recovery work happen in coordination with each other, managed by the same clinical team.

Bridge of Hope works with many insurance providers to help make treatment as accessible as possible. We accept most private insurance plans as well as Oregon Medicaid through CareOregon and Trillium.

We do not accept Medicare at this time.

The next step is a short pre-screen phone call, where we ask a few questions about your substance use history, mental health, living situation, medical considerations, and current circumstances. This helps us determine whether our program is a good fit. You will typically know within a few hours, and always by the end of the day, whether you are approved to move forward.

If approved, we schedule a clinical assessment, which helps determine the appropriate treatment plan. Bridge of Hope provides outpatient treatment, and many clients also move into sober living while participating in the program. Some individuals choose to complete an assessment first and then decide on next steps based on the recommendations.

Yes. Flexibility is a foundational part of how outpatient treatment works, and it is one of the biggest reasons people choose it over residential care. All of our programs, from HIOP down to standard outpatient, are structured so that clients can continue meeting their real-world responsibilities. Even at the most intensive level, you return home or to sober housing each evening. Your treatment team will work with you to build a schedule that fits your work hours, childcare needs, or school commitments without compromising the quality of your care.

Yes to both. Our LGBTQ+ Programs are built around the understanding that affirming, identity-conscious care leads to better outcomes. We provide a safe, inclusive environment where clients can engage fully in their recovery without fear of judgment or discrimination. We also operate a dedicated LGBTQ+ sober living house for clients who need housing support during treatment.

For individuals navigating a DUII charge, our DUII Treatment program is specifically structured to meet Oregon court and DMV requirements while keeping the focus on genuine, lasting recovery and not just compliance. Our Community-Based Recovery Programs provide additional layers of support for clients who benefit from peer connection and community-based resources alongside clinical care.

Yes. In addition to counseling and recovery groups, Bridge of Hope incorporates recreational outings and community activities that support connection, healthy routines, and positive experiences in recovery. These outings give clients the opportunity to build relationships, practice sober socializing, and reconnect with the community.

Examples of past outings include visits to OMSI, the Oregon Zoo, Powell’s Books, and the Portland Art Museum. Clients have also enjoyed outdoor activities such as kayaking on the Willamette River, swimming and BBQs at Frenchman’s Bar at Cline Line Park in Vancouver, trips to Multnomah Falls, and fishing at the Sandy Trout Farm. Other social activities may include group outings like pizza and arcade nights. Activities vary throughout the year and are designed to support wellness and community building during recovery.

Bridge of Hope offers a combination of evidence-based treatment, holistic practices, and individualized counseling to support long-term recovery. Clients participate in structured group sessions as well as one-on-one counseling with a licensed counselor for one hour each week.

Evidence-based groups may include topics such as grief and loss, process groups, anger management, and healthy boundaries.

We also incorporate holistic and creative approaches to recovery. These may include yoga, art therapy activities such as painting or making custom t-shirts, and Wellbriety programming rooted in Native American and Indigenous recovery traditions. Wellbriety activities can include cultural crafts such as loom work, drum making, and beading.
